Member may cancel this agreement by providing a signature four business days before the date of withdrawal (15th of each month). Oral cancellation, cancellation by phone, or email will not be accepted. Only the Member may cancel the agreement regardless of credit/debit/checking/savings account information on file. Member signature at cancellation must match Member signature at sign up. Member will receive no refunds for services unused or for lack of attendance by Member.

Avon Fitness requires a signature for cancellation of autobill memberships. Signatures must be received four business days before the date of withdrawal (15th of each month). If you are unable to visit the gym to sign the cancellation form, it can be mailed to you, or emailed requesting a form to provide an e-signature.
